The reform is realized by the internal and external actors acting together in the process. In this process, neither internal actors nor external actors are decisive in their own right. However, this should not mean that both actor groups act together in the formation of reforms. In some reform implementations, the impact of internal actors is more pronounced, in some cases the impact of external actors is more pronounced. The city hospitals, which emerged as an important reform application in the health system, came to life with the effect of internal and external demands. This study takes this reform application which is very important in health system as a research object. However, in the study, it was not the internal dimension that was effective in the life of city hospitals; focus on the external dimension. The thesis of the study appears at this point and it is claimed that international organizations have a very important position in the birth of the city hospitals.
